Financial Aid Verification Forms
This is for students whom have been selected for financial aid verification after completing their FAFSA. If you have been selected you will receive a letter in the mail. Those selected for verification must fully complete and return it to this office along with the following:

| Independent Verification Form completed & signed | Depentent Verification Form completed & signed by both student and parent/gardian |
| Other: documentation for untaxed income received in that l year | Other: documentation for untaxed income received from student or parents/gardian |

In order to receive federal financial aid, you must submit the above information no later than three weeks from the date of this letter.
Please be advised that the school will not disburse any Federal Pell, FSEOG, TSAA< Wilder-Naifeh Technical Skills Grant, or start FWS employment until you have cmpleted the verifiction process.
Also, applicants who fail to provide the requested documentation to verify reported information within the three weeks' time period will forfeit their financial aid award.
