Proof of MMR and Varicella Immunization
Pursuant to Tennessee Department of Health policy regarding immunization against certain diseases prior to school attendance in Tennessee, the Tennessee Technology Center at Oneida/Huntsville (TTCO/H) will begin requiring proof of immunization for measles, mumps, and rubella (MMR) beginning January 1, 2010. Additionally, proof of two doses of the Varicella vacine, laboratory evidence of immunity, or a history of varicella disease from a health practitioner will become a requirement on July 1, 2011.
Any new student enrolling at TTCO/H must provide proof of the first and second dose of the MMR and Varicella vaccines prior to being placed on the official waiting list. Subsequently, students must provide proof of the second dose of the MMR vaccine no later than the first day of the students' next term of enrollment. Students will not be accepted for enrollment period if they fail to provide proof of the MMR vaccinations.
Certain students may be exempt from receiving these vaccinations if they can provide the requested forms as specified on the sheet that is attached below.
The file below needs to be printed out and completed by your physician. Form must be returned to Student Services.
