LPN Application Enrollment Packet
All applicants interested in the Practical Nursing Program (LPN) must pass the COMPASS Entrance Test for admission into the Practical Nursing program. We strongly encourage you to enroll in a class to reveiw you basic reading and math skills prior to taking this exam. Please contact Caitlyn Reneau for free tutoring classes at our Oneida campus.
In addition to the enrollment application you must also comply with state regulation of immunizations. Any new student enrolling at TTCO/H must provide proof of the first and second dose of the MMR and Varicella vaccines prior to being placed on the official waiting list. Subsequently, students must provide proof of the second dose of the MMR vaccine no later than the first day of the students' next term of enrollment. Students will not be accepted for the enrollment period if they fail to provide proof of the MMR vaccinations.
Both forms must be completed.
